ARTICLE 1:

• The Customer is provided information in detail and consulted technique on cosmetic tattoo and fibroblast, removal in LUXX BEAUTY ML.
• LUXX BEAUTY ML must comply with the technical implementation in conditions of hygiene and health safety.
• All instruments and tattoo ink are new, used separately for each person and not reused.
• Price of the services, incentive program is agreed and have unity of the two parties. ­
• The Contract includes Terms of Services, Policy as the part of Consent form.

• The customer agrees to shoot and use the photo for advertising purposes. Except the customer uses breast tattoo service, we are committed to not disclose identity of the customer.

ARTICLE 4:

• HEALING TAKES AROUND 4 WEEKS FOR EYEBROWS AND EYELINERS, 3 MONTHS FOR LIPS
• Pigments are carefully selected to suit each individual skin tone and natural hair colour. Pre-drawing is completed prior the procedure/ and tattooing will only begin once shape, style and colour have been agreed upon
• Our prices are for clients with no existing cosmetic tattoo, additional charges may apply if a correction service is required
• Imbalance in the acceptable level of eyebrows, eyelids, lip liner.
• Colours can be fainter or darker 1 tone than the original request.
• Swelling, slight irritation, itching or turning red can occur. According to our experiences, these symptoms are only temporary for a few hours/ days after using cosmetic tattooing.
• In the first seven days, eyebrows are darker than 40% and thicker than 10 – 15%. Colours and reflections of colour depend on skin’s natural pigment. Shape is determined by the face ratio. Symmetry is determined by a digital method, and the customer must close their eyes to avoid influence of expression on their face. Inkjet ink is absorbed differently due to various skin type, so it cannot guarantee success in absolute level.
• Cosmetic Tattooing is a TWO (2) step process and the second session must be attended 4 weeks post initial treatment additional charges may apply if after 4 weeks. In rare circumstances, additional visits may be required for optimal results and charges may apply.
• Efficiency time (durable time) at minimum or maximum of the cosmetic tattooing cannot determined precisely and also cannot be guarantee for each execution.
• Editing (finishing) of the 1^sttimes will take place 1 month after performing eyebrows tattooing, and 3 months for lips. For oily skin, the editing is really necessary.
• Cosmetic tattooing often leads to damage to the skin. Therefore, skin care carefully and slightly after treatment is very important for healing the skin and does not leave complications. Inadequate care of skin healing process can lead to bad results and LUXX BEAUTY ML is not responsible for this.
• This is semi-permanent treatment meaning my body will naturally break down the pigment over time, therefore, maintenance may be needed every 6 - 8 months or maintenance annually to keep the shape and colour refreshed.

ARTICLE 5:

• I understand that a certain amount of discomfort is associated with this procedure and that swelling and redness may occur. They will dissipate within 24 hours or after 2 days.
• I understand that all procedures carry a risk of infection and agree to contact the technician immediately should there be any concerns.
• I accept responsibility for the aftercare of my treatment and agree to strictly follow the guidelines I have been given, using only the cream recommended.
• I consent to the practitioner applying Topical Anaesthetics containing Lignocaine and Tetracaine Prilocaine.
• I understand that any skin treatments i.e. Retin A, Renova, Alpha Hydroxy and Glycolic Acids, laser hair removal, plastic surgery, or other skin altering procedures may result in adverse changes to my permanent makeup.
• I understand that sun, tanning beds, pools, some skin care products and medications can affect my permanent makeup.
• I understand this procedure/s carries with it, known and unknown complications and consequences including but not limited to: infection, allergic reaction, scarring, inconsistent colour, and spreading, fanning or fading of pigments.
• I understand that this procedure/s is for cosmetic purposes only and that no guarantees have been made to me regarding the results. Skin quality (age, thick epidermis and excessively oily skin), general health, post treatment and exposure to various external factors may result in poor healed results.
• I agree that I cannot give blood for 4 months after a tattooing and/or removal treatment as per Australian health regulations.
• I understand that successful colour saturation can NOT be guaranteed due to hidden scar tissue
• I will tell all skin care professionals or medical personnel about my permanent makeup procedures
• I accept the responsibility of explaining to you my desire for specific colours, shape, and position for any procedure done today
• I understand that implanted pigment colour can slightly change or fade over time due to circumstances beyond your control and I will need to maintain the colour with future applications.
• I grant permission to LUXX BEAUTY ML to take and use: photographs and/or digital images of me for use in news releases, educational materials and/or social media platforms including but not limited to Instagram, Facebook, and Pinterest
• I acknowledge that the proposed procedure(s) involve risks inherent in the procedure and have possibilities of complications during and/or following the procedures such as: infection, poor colour retention and hyper-pigmentation
• If I have any signs and symptoms of infections, I will seek medical care. These include but are not limited to: redness, swelling, tenderness of the procedure site, red streak going from procedure site towards the heart, elevated temperature, or purulent drainage from the procedure site.
• I understand that after my service, there will be no refunds. No exceptions
• I acknowledge that this procedure may alter my appearance and that no representations have been made to me as to remove my permanent makeup. To my knowledge I do not have a physical, mental, medical impairment or disability which might affect my wellbeing as a direct or indirect result of my decision to have permanent makeup.
• I acknowledge it is not reasonably possible for my technician to determine whether I might have an allergic reaction to the pigments or ointment used in this process. I agree to accept the risk that such reaction is possible.
• I understand the restrictions on physical activities such as bathing, recreational water activities, gardening, or contact with animals, and the duration of the restrictions.
• I have read and followed the Pre-care guidelines and acknowledge that the Aftercare instructions will be explained and provided to me which I agree I am responsible for and if not followed can impact my results negatively.
• I certify that I have read or have had read to me the contents of this whole form. I understand the risks and alternatives involved in this procedure(s) and I have had the opportunity to ask questions and all of my questions have been answered. I acknowledge that I have reviewed and approved the material given to me. This is true and accurate of my medical history, past and present. I am aware that failure to disclose information pertinent to my treatment could have serious health ramifications. I am also aware that failure to disclose information pertinent to my treatment could have a direct bearing on treatment outcome. I am not under influence of alcohol or illegal drugs, or pregnant.
• For all reasons above, I fully understand and accept that there will be no refunds from Luxx Beauty ML Studio to client and no guarantee specific results because the results will depend on various of body chemistry, skin condition, life style and after-care.
• I fully understand and accept that I will NOT take any legal actions under any circumstances against PMU Artists of Luxx Beauty ML Studio and Studio from now on.
